Kindergarten - Sixth Grade Student Enrollment Requirements
Before a child can officially enroll the following must be provided:
- Kentucky Immunization Certificate
- A Valid Birth Certificate
- A Physical Examination
- A Vision Examination
- A Dental Examination
- Proof of Residency (2)
If your child has a social security number, please bring a copy to pre-registration.
